- CMV-seropositive kidney transplant recipients
- Must have a negative pregnancy test (non-menopausal women)

- HIV-infected patients
- Patients with multivisceral transplants
- Those who are not expected to receive universal prophylaxis
- Receiving induction therapy with anti-thymocyte globulin (ATG)
